What is tirzepatide used for?

Tirzepatide is an injectable medication mainly used for:

  1. Treatment of type 2 diabetes
It provides very effective blood-glucose control.

  2. Weight loss / obesity management
It is prescribed for people with obesity or overweight with related risk factors, leading to significant and sustained weight reduction.

It is considered a next-generation medication because it targets two metabolic hormones at the same time, making it more potent than similar drugs.

How does it work?

Tirzepatide is a dual GIP and GLP-1 receptor agonist (incretin hormones):

  • GLP-1

    • Reduces appetite

    • Slows gastric emptying

    • Lowers blood sugar levels

  • GIP

    • Improves insulin response to glucose

    • Enhances the effect of GLP-1

    • Improves insulin sensitivity

Combined effect

👉 Less hunger
👉 Longer-lasting satiety
👉 Better blood sugar control
👉 Progressive weight loss

How is it used?

💉 Administration

  • Subcutaneous injection

  • Once per week

  • Injection sites:

    • Abdomen

    • Thigh

    • Back of the upper arm

It comes in pre-filled injection pens, similar to insulin pens.

📈 Typical dosing schedule (step-up)

To minimize side effects, treatment starts at a low dose and is increased gradually:

  1. 2.5 mg / week → starting dose (adaptation phase)

  2. 5 mg / week

  3. 7.5 mg

  4. 10 mg

  5. 12.5 mg

  6. 15 mg (usual maximum dose)

⏱️ Dose increases are typically done every 4 weeks, depending on tolerance.

⏳ When do effects appear?

  • Appetite reduction: within 1–2 weeks

  • Blood glucose improvement: days to weeks

  • Weight loss: noticeable after the first 1–2 months

Many patients lose 15–25% of their body weight with long-term use under medical supervision.

⚠️ Most common side effects

Especially at the beginning or when increasing the dose:

  • Nausea

  • Vomiting

  • Diarrhea or constipation

  • Feeling full very quickly

  • Acid reflux

🔹 These usually decrease over time
🔹 Eating slowly and avoiding high-fat meals helps a lot

🚫 Who should NOT use it?

  • People with:

    • Medullary thyroid cancer (personal or family history)

    • MEN type 2 (Multiple Endocrine Neoplasia)

    • Active pancreatitis

  • Pregnancy or breastfeeding

  • Use without medical supervision

⚖️ How it differs from similar medications

  • More effective for weight loss than semaglutide

  • Stronger blood-glucose control

  • Acts on two hormonal pathways, not just one
